To mark this year精东影业 American Independence Day, Alaska Airlines unveiled its latest patriotic aircraft design and Fallen Soldier Cart.

On the 4th of July, the airline revealed its fourth 鈥楬onoring Those Who Serve鈥 aircraft and 16th Fallen Soldier Cart. 

The B737 MAX 8 aircraft was specially designed in partnership with Boeing. It features a patriotic paint scheme with symbolic imagery representing all six branches of the US Armed Forces: Air Force, Army, Navy, Marine Corps, Coast Guard, and Space Force.

鈥淔rom its star-spangled design to the heartfelt message emblazoned on its side, this aircraft is a flying tribute to the courage and dedication of those who serve today, and those who served in the past,鈥 Alaska Airlines said in a statement.

The airline added that, when the aircraft is seen in the skies, it will serve as a reminder of the sacrifices made by US military personnel.

In addition to the newly designed aircraft, Alaska Airlines delivered its 16th Fallen Soldier Cart to Nashville International Airport (BNA) on June 26, 2024.

Fallen Soldier Carts, designed and crafted by Alaska精东影业 Maintenance and Engineering department, are described by the airline as a solemn and respectful symbol that transports the remains of fallen service members, ensuring that they are treated with the utmost dignity and respect. 

On its first mission, the new cart transported the cremated remains of US Navy Vietnam veteran and Aviation Boatswain精东影业 Mate, Roy Campbell. 

The Fallen Soldier Program was created in 2011 to ensure that military protocols are followed when handling the remains of fallen service members being flown to their final resting place on Alaska Airlines. 

鈥淭his cart begins its most important duty today鈥攄oing what is on the side of our beautiful liveries鈥攈onoring those who serve, transporting our fallen heroes home to their final resting place with honor and dignity,鈥 said Carlos Zendejas, Horizon VP of flight operations and Air Force veteran.

聽鈥淥ur hope is that this cart will also bring comfort to the family, friends and loved ones during that very difficult time,鈥 Zendejas added.

Comments made by a Boeing executive during a media briefing that irked the NTSB included the claim that paperwork issues were to blame for the Alaska Airlines MAX 9 door plug blowout on January 5, 2024. 

On June 27, 2024, the National Transport Safety Board (NTSB) castigated Boeing after discovering an executive disclosed non-public information regarding the 737-9 accident investigation to members of the media at a briefing two days previously.听听聽

The NTSB claims that Boeing 鈥渂latantly violated鈥 investigative regulations, as well as an agreement signed by the planemaker not to share information privy to the probe.聽

鈥淏oeing offered opinions and analysis on factors it suggested were casual to the accident,鈥 the NTSB said in its statement.    

In response to the violation, the NTSB said Boeing will 鈥渘o longer have access to the investigative information the NTSB produces as it develops the factual record of the accident鈥.   

According to , an angry letter was also sent to Boeing CEO Dave Calhoun later in the day from NTSB Office of Aviation Safety Director Timothy LeBaron.聽

LeBaron wrote that the executive’s comments during the media briefing at Boeing精东影业 Renton facility “released non-public investigative information and made unsubstantiated speculations about possible causes of the Jan. 5 door plug blowout鈥. 

He added: “We have verified that part of the released information was either inaccurate or unknown to the NTSB while other parts were not previously disclosed to the public.” 

The NTSB director also reaffirmed that it decided not to remove Boeing as a party to the door plug probe, though any further violations could lead to that happening.听听

What the Boeing executive said during the media briefing聽

In Timothy LeBaron精东影业 letter to Dave Calhoun, he confirmed the executive that spoke to the media was Elizabeth Lund, the Senior Vice President of Quality for Boeing精东影业 Commercial Airplane Unit. 

According to , Lund told the group of journalists that a lack of paperwork was the reason why four bolts vital to holding the 737-9 door plug in place were not reinstalled during the plane精东影业 production.听听聽

added that Lund said a 鈥渕ove crew鈥 reinstalled the plug door after it was removed, so maintenance work could be carried out on some rivets in the cabin, but they 鈥渄id not reinstall the retaining pins鈥.聽

鈥淭hat is not their job. Their job is to just close it, and they count on existing paperwork,鈥 Lund said.  

Journalists at the briefing were also told that Boeing 鈥渂elieves that plug was opened without the correct paperwork鈥. 

Lund said: 鈥淭he fact that one employee could not fill out one piece of paperwork in this condition and could result in an accident was shocking to all of us.鈥   

鈥淲e know the plug was opened, and the paperwork is not there,鈥 she added. 

Paperwork issues at Boeing were not themself anything new in relation to the door plug probe, but Lund fleshed out more alleged details than had been previously heard.  

Responding to Lund精东影业 briefing LeBaron wrote in his letter that only 鈥淣TSB personnel are authorized to publicly disclose investigative information and, even then, the disclosure is limited to factual information verified during the course of the investigation鈥.  

In a statement provided to CNN on June 27, 2024, Boeing said: 鈥淲e deeply regret that some of our comments, intended to make clear our responsibility in the accident and explain the actions we are taking, overstepped the NTSB精东影业 role as the source of investigative information. We apologize to the NTSB and stand ready to answer any questions as the agency continues its investigation.鈥

The National Transport Safety Board (NTSB) has castigated Boeing after discovering officials disclosed non-public information regarding the Alaska Airlines 737 MAX 9 plug door investigation to members of the media. 

According to a statement by the NTSB on June 27, 2024, Boeing 鈥渂latantly violated鈥 investigative regulations and an agreement signed by the planemaker not to share information privy to the probe or speculate about causes of the 737-9 door plug blowout on January 5, 2024.听听

The NTSB said that on June 25, 2024, Boeing held a media briefing at its facility in Renton to talk about quality improvements but during the event a company executive 鈥減rovided investigative information鈥 and gave an 鈥渁nalysis of factual information already previously released鈥. 

鈥淏oth of these actions are prohibited by the party agreement that Boeing signed when it was offered party status by the NTSB at the start of the investigation. As a party to many NTSB investigations over the past decades, few entities know the rules better than Boeing,鈥 the NTSB wrote in a scathing statement. 

The NTSB added that during the media briefing Boeing portrayed the NTSB investigation as a 鈥渟earch to locate the individual responsible for the door plug work鈥.听听

‘Few entities know the rules better than Boeing’

The NTSB has been at pains to ensure employees at Boeing know that it is not a witch-hunt and that it is only focused on the 鈥減robable cause鈥 of the accident without placing blame on any individual.  

After the NTSB learned of the violation, Boeing provided a transcript of the media briefing which revealed that the executive had shared information that the safety board had not 鈥渧erified or authorized for release鈥. 

鈥淏oeing offered opinions and analysis on factors it suggested were casual to the accident,鈥 the NTSB said in its statement.  

Following the disclosure, the NTSB announced several restrictions and sanctions against Boeing.  

The NTSB said that Boeing will retain its party status but will 鈥渘o longer have access to the investigative information the NTSB produces as it develops the factual record of the accident鈥. 

鈥淭he NTSB will also subpoena the company to appear at an investigative hearing into the case scheduled for Aug 6 and 7 in Washington, DC. Unlike the other parties in the hearing, Boeing will not be allowed to ask questions of other participants,鈥 the NTSB said. 

The NTSB confirmed that it will advise the (DOJ) about the unauthorized disclosure.听听

“Given that Boeing is under investigation by the U.S. Department of Justice in relation to its Deferred Prosecution Agreement stemming from Boeing精东影业 interactions with the FAA prior to the Boeing 737 MAX fatalities, the NTSB will be coordinating with the DOJ Fraud Division to provide details about Boeing’s recent unauthorized investigative information releases in the 737 MAX 9 door plug investigation,” the safety board said. 

Boeing is waiting to hear what punishment it will face from the DOJ for violating its 2021 deferred prosecution agreement put in place following the 737 MAX fatal crashes in 2018 and 2019.聽 聽聽聽聽

Alaska Airlines has relaunched a nonstop service between Anchorage and New York in the United States (US) for the summer season.  

From June 13, 2024, flights between John F. Kennedy International Airport (JFK) for Ted Stevens Anchorage International Airport (ANC) will restart, connecting Alaska with the Big Apple.  

While operating over the summer, the route between New York and Anchorage will become Alaska Airlines‘ longest route on its network.  

Alaska has scheduled the 3,385 mile route with its new Boeing 737 MAX 8 fleet, with the flight expected to take around seven hours.  

According to Alaska the new seasonal route, which will operate until August 19, 2024, is the only nonstop service between JFK and ANC. 

鈥淎laskans love travel options and love them even more when they include nonstop flights,鈥 Marilyn Romano, regional vice president of Alaska for Alaska Airlines, said. 鈥淥ur new service to New York JFK is a perfect addition to our flight schedule for Alaskans wanting some adventure in one of the world精东影业 most dynamic cities, and for New Yorkers eager for new experiences in our great state.鈥   

An overnight flight will depart JFK at 09:45 and arrive in Anchorage at 13:30. The return flight will depart ANC at 20:00 and arrive in New York at 07:05 +1.听听

During the summer, Alaska Airlines will also be flying non-stop from to Chicago, Denver, Honolulu, Los Angeles, Las Vegas, Minneapolis, Phoenix, Portland, Salt Lake City, San Diego, San Francisco, and Seattle.

A former quality manager at Spirit AeroSystems, and now whistleblower, has alleged that 737 MAX fuselages were sent on to Boeing with serious defects.  

Santiago Paredes, who worked for Spirit AeroSystems between 2010 and 2022, told the and in an exclusive interview that he would find 鈥渁nywhere from 50 to 100, 200鈥 defects (some more serious than others) on fuselages that were being prepared to be sent to Boeing.听听

“I was finding a lot of missing fasteners, a lot of bent parts, sometimes even missing parts,鈥 Paredes said during his interview.  

What精东影业 more Paredes claimed that he felt under pressure to be less thorough with his inspections and even earned the nickname, 鈥渟howstopper鈥. 

鈥淭hey just wanted the product shipped out. They weren鈥檛 focused on the consequences of shipping bad fuselages. They were just focused on meeting the quotas, meeting the schedule, meeting the budget鈥 If the numbers looked good, the state of the fuselages didn鈥檛 really matter,鈥 Paredes claimed.聽

‘Working at Spirit, I almost grew a fear of flying’

Paredes also told CBS that during his time at the Spirit facility in Wichita, Kansas, he often found problems around the same aircraft door plug that was separated from an Alaska Airlines 737 MAX 8 jet on January 5, 2024. 

“Working at Spirit, I almost grew a fear of flying. Knowing what I know about the 737, it makes me very uncomfortable when I fly on one of them,” Paredes told NBC.  

In his interview with the BBC, Paredes alleged that Boeing knew Spirit were sending fuselages with defects.   

According to the BBC, Paredes was eventually told by one of his seniors to alter the way he reported defects, thus reducing the overall number.  

After objecting, the former US Air Force airman was demoted and relocated to another area at the factory.  

Paredes was subsequently reinstated after he filed an “ethics complaint” with Spirit, which was partially upheld.聽

‘My last cry for help’

Paredes wrote in an email to the Spirit CEO Tom Gentile at the time and said: “I have lost faith on the quality organization here at Spirit and this is my last cry for help.鈥  

Despite being reinstated to his leadership role Paredes, who now says he is reluctant to fly on a 737 MAX, decided to leave Spirit.  

In a statement Spirit told the BBC that it “strongly disagree[d]” with the allegations while Boeing told CBS that it had long had a team in place that finds and fixes defects in fuselages that arrive from Spirit.  

A spokesperson for Boeing added that since March 2024, engineers for the company have been checking all fuselages at the Spirit factory as they come off the production line.听听聽聽

Spirit shareholder lawsuit

Paredes’ allegations are part of a lawsuit against Spirit by shareholders in which he claims there were “widespread quality failures” at the company. Spirit told CBS the allegations are “unfounded.” 

Boeing is understood to be in talks to buy back Spirit, which was once part of the same organization. 

Since the Alaska Airlines plug door blowout on January 5, 2024, Boeing and Spirit have both been criticized by the Federal Aviation Administration (FAA). 

As part of an audit of the two companies following the Alaska accident the FAA said it found 鈥渕ultiple instances where the companies allegedly failed to comply with manufacturing quality control requirements鈥.   

The plug door on the Alaska Airlines 737-9 was fitted by Spirit but was subsequently removed by Boeing staff during assembly for further maintenance work.听听

United States (US) Senator and pilot Tammy Duckworth has written to the Federal Aviation Administration (FAA) to ask the agency to examine why crew aboard the Alaska Airlines 737 MAX 9 that suffered a door plug blow out on January 5, 2024, were unaware that the cockpit door was designed to open automatically during a rapid depressurization. 

In a letter to FAA chief Michael Whittaker on April 4, 2024, Duckworth said that the 鈥渦nknown, undisclosed feature鈥 surprised the crew and resulted in an emergency checklist and a pilots鈥 headset being sucked out of the cockpit during the emergency. 

News that the cockpit door had flung open by itself during the Alaska Airlines 737-9 door plug blowout incident came to light when the National Transportation Safety Board Chair (NTSB) Jennifer Homendy spoke with reporters at a press briefing on January 8, 2024.听听

Homendy said: 鈥淭he cockpit doors flew open immediately and at this point the flight attendant in the forward portion of the aircraft was standing. The cockpit door flew open, hit the lavatory door. The lavatory door got stuck. She did attempt to shut the door three times. It eventually shut, but it did blow open during the explosive decompression.鈥 

In her letter to the FAA, Duckworth declared that it is critical that the flight crew are 鈥渇ully informed of all features on the flight deck鈥. 

Duckworth said this was the third time that Boeing pilots have been kept in the 鈥渄ark鈥 on MAX aircraft.听听

‘Boeing精东影业 failure to disclose this feature is chilling’

鈥淏oeing精东影业 failure to disclose this feature is chilling given its history of concealing 737 MAX information from pilots,鈥 Duckworth wrote.  

Citing the first example, Duckworth said in a press release that Boeing decided to 鈥渘ot include the Maneuvering Characteristics Augmentation System, MCAS, in the 737 MAX 8 flight manual, which left the flight crew on Lion Air flight 610 unaware of what was forcing their aircraft into a nose-dive”.聽

Secondly, she said that Boeing decided to 鈥渘ot inform airlines or pilots that the Angle of Attack (AOA) disagree alert aboard most 737 MAX 8 jets was nonfunctional鈥. 

鈥淭his is dangerous, and the FAA must not view this latest omission in isolation. Instead, FAA should consider regulatory action informed by Boeing精东影业 past pattern of deceptive conduct in developing and delivering the MAX series of aircraft and a strong commitment to avoid repeating past FAA mistakes in deferring to Boeing,鈥 Duckworth wrote in her letter to the FAA.  

Duckworth advised the FAA to work with the NTSB to discover why the Alaska Airlines crew were unaware the cockpit door would open during a rapid decompression.听听

鈥淭he FAA also needs to work with NTSB to consider whether other elements of the cockpit might need improvement in light of the previously undisclosed cockpit door design. Finally, as noted above, FAA must take into account that this is the third time Boeing has failed to disclose a flight deck feature to 737 MAX pilots,鈥 Duckworth精东影业 letter concluded.  

In a statement to in response to Duckworth精东影业 call, Boeing said that it was “committed to continued transparency and sharing information with our regulator and operators鈥.聽

The National Transportation Safety Board (NTSB) chair Jeniffer Homendy has said in a letter to the United States Senate Commerce Committee that she has still been unable to ascertain who 鈥減erformed the work to open, reinstall, and close the door plug鈥 on the Alaska Airline Boeing 737 MAX 9 that suffered a door plug blowout on January 5, 2024.  

On March 7, 2024, Homendy appeared in front of the committee and said that documentation which recorded the removal and reinstallation of the door plug during repair work to the 737-9 at the Boeing factory on September 19, 2023, had not been provided by the planemaker.听听聽聽

Homendy also said attempts to take the names of 25 Boeing employees who may have worked on the door plug removal and reinstallation had so far been fruitless.   

At the hearing senators told Boeing to provide the NTSB with the information it needs and Homendy was instructed to advise the committee in writing of the outcome.  

In a addressed to Senator Maria Cantwell, the chair of the committee, on March 13, 2024, Homendy stated that Boeing had advised her that it was 鈥渦nable to find the records documenting鈥 the work carried out on the door plug and that security camera footage that could help obtain this information had been overwritten.聽

鈥淭he absence of those records will complicate the NTSB精东影业 investigation moving forward,鈥 Homendy wrote. 

The names of the 25 staff members who reported to the door crew manager in September 2023, were subsequently provided by Boeing following the Senate Committee. However, the identity of those that specifically worked on the door plug was not singled out.  

鈥淎fter NTSB received this list, I called Boeing Chief Executive Officer David Calhoun and asked for the names of the people who performed the work. He stated he was unable to provide that information and maintained that Boeing has no records of the work being performed,鈥 Homendy wrote in her letter.聽

Moving forward聽with investigation

Increasingly, concerned that it may be perceived that the NTSB is seeking to find out the names for 鈥減unitive purposes鈥, Homendy wrote in her letter that that she only wished to 鈥渟peak with them to learn about Boeing精东影业 quality-assurance processes and safety culture鈥. 

鈥淥ur only intent is to identify deficiencies and recommend safety improvements so accidents like this never happen again. In fact, our nation精东影业 aviation record is so safe precisely because of our well-established culture of non-punitive reporting,鈥 Homendy wrote. 

Accordingly, Homendy advised the committee that she has instructed the NTSB to 鈥渦tilize our authority to protect the identities of the door crew and other front-line employees who come forward with information relevant to the investigation鈥. 

She added that she did not want the focus on individual names to 鈥渘egatively impact our investigation and discourage鈥 Boeing employees from coming forward. 

鈥淭he NTSB investigation, as with all our investigations, seeks to determine the probable cause of this accident and issue safety recommendations to prevent it from reoccurring. Our only goal is to ensure the increased safety of the flying public,鈥 Homendy concluded in her letter.聽

The National Transportation Safety Board (NTSB) will hear sworn testimonies at a two-day investigative hearing into the Alaska Airlines Boeing 737 MAX 9 door plug blowout on January 5, 2024.  

The investigative hearing, announced on March 12, 2024, will hear from witnesses as the NTSB gathers evidence to 鈥渄etermine the facts, circumstances, and probable cause of the transportation accident鈥.聽

The hearing, which takes place on August 6 and 7, 2024, will be broadcast through a livestream and is open to the public, however, only NTSB board members, investigators and scheduled witnesses can participate.  

The NTSB is authorized to compel testimony, through use of a subpoena, if necessary. 

According to the NTSB精东影业 website, witnesses will be called if they hold information that the safety board has not already discovered, needs clarification or 鈥渟hould be publicly vetted in an open forum鈥.聽

A six-week audit, carried out by the Federal Aviation Administration (FAA) of Boeing following the door plug incident found that out of 89 product audits the planemaker passed 56 tests and failed 33, according to the New York Times.聽 聽

The article精东影业 findings were based on an internal FAA slide presentation sourced by the publication. 

Spirit AeroSystems facilities were also subjected to the FAA audit of which, out of 13 product audits, seven failed.  

The slide presentation included an instance in which a Spirit employee reportedly used a hotel key card to check a door seal. 

In response to the audit, Boeing’s Head of Commercial Aviation Stan Deal told employees in a leaked memo that most of the instances of non-compliance 鈥渋nvolved not following our approved processes and procedure鈥.聽

“We have used your feedback, and those from our regulator and customers, to take immediate actions to strengthen our safety and quality. These actions are central to a comprehensive plan we will soon deliver to the FAA,鈥 Deal wrote. 

Boeing has been given 90 days to respond to the FAA audit and a secondary 50-page report compiled by an expert panel that was published on February 26, 2024.聽

A lawyer representing 22 people onboard the Alaska Airlines flight that suffered a rapid decompression on January 5, 2024, has alleged in a lawsuit that passengers previously flying on the same Boeing 737 MAX 9, registered N704AL, reported hearing a 鈥渨histling sound鈥 coming from the 鈥渧icinity鈥 of the door plug.  

On February 7, 2024, Mark Lindquist, amended a previous lawsuit he filed alleging emotional and physical injuries to passengers, including severe stress, anxiety, trauma, and hearing damage, to include new accusations against both Alaska and Boeing. 

“There was a whistling sound coming from the vicinity of the door plug on a previous flight of the subject plane,鈥 the claim read. 鈥淧assengers apparently noticed the whistling sound and brought it to the attention of flight attendants who reportedly informed the pilot or first officer.鈥 

According to a statement from Lindquist, no known further action was taken 鈥渁fter the pilot checked cockpit instruments, which purportedly read normal”. 

Linquist also cited the cockpit door that flung open during the rapid cabin depressurization, a design feature that the Alaska pilots were unaware of. 

“The resulting shock, noise, and communication difficulties contributed to a lack of proper communication between the flight crew and passengers, thereby intensifying confusion and stress,” according to the lawsuit. 

Linquist claimed Boeing should have fixed their quality control issues following the two fatal 737-8 crashes in 2018 and 2019. 

On February 6, 2024, the (NTSB) published its preliminary investigation report indicating four bolts to hold the mid-exit plug door in place were missing.听听

On January 5, 2024, Alaska Airlines flight 1282 took off from Portland International Airport (PIA), but as it approached 16,000 feet (about 5 kilometers), a plug door on the left-hand side of the fuselage separated from the aircraft.  

“This plane was a ticking bomb,” Lindquist said. “A blowout could have happened at a cruising altitude where it would have been catastrophic.”聽

The US National Transportation Safety Board (NTSB) has published its preliminary investigation report into the Alaska Airlines Boeing 737 MAX 9 plug door blowout incident.聽

The 19-page document details how events unfolded on January 5, 2024, but perhaps more critically the report focuses on what happened before the 737-9 left Boeing精东影业 Renton facility for delivery to Alaska Airlines

The preliminary report confirmed that four key bolts designed to hold the plug door in place were missing.  

鈥淔our bolts that prevent upward movement of the mid exit door plug were missing before the mid exit plug moved upward off the stop pads,鈥 the NTSB said.  

The NTSB drew this conclusion partly because investigators 鈥渙bserved damage patterns and absence of contact damage or deformation鈥 around holes associated with the four bolts which suggested that they were not in place during the incident. 

This chimed with an earlier report from a person purporting to be a whistleblower at Boeing who wrote that the bolts were 鈥渘ot installed when Boeing delivered the airplane鈥.     

With the evidence suggesting that the four bolts were not present when the plug door separated from the Alaska 737-9, the logical question to ask is why they were not there.  

The plug door itself was manufactured by Spirit AeroSystems Malaysia on March 24, 2023, and was received at Spirit AeroSystems Wichita on May 10, 2023. 

According to records, the Alaska fuselage with the door plug installed arrived at Boeing精东影业 Renton facility by rail on August 31, 2023.   

On September 1, 2023, a Non-Conformance Record (NCR) was created noting 鈥渇ive damaged rivets on the edge frame forward of the left plug鈥. 

The NTSB said in its report that to fix the five rivets the plug door was opened, and the four bolts were removed.

Records obtained by the NTSB show that the rivets were replaced and fixed on September 19, 2023, by Spirit AeroSystems workers. 

Later that same day at 6.39 pm an image of the plug door closed was shared by a Boeing employee to a colleague via text while they were discussing interior restoration following the repair work.  

According to the NTSB the photo was taken before interior restoration and after the rivet rework was completed. 

The photo shows there are no bolts in the three visible locations, while the fourth is covered with insulation. 

鈥淭he investigation continues to determine what manufacturing documents were used to authorize the opening and closing of the left MED plug during the rivet rework,鈥 the NTSB said in the report. 

The NTSB added that after the plane left the Boeing factory to the time of the accident it 鈥渇ound no evidence that the left plug was opened after leaving Boeing精东影业 facility鈥.   

Boeing response to report 

Following the report being published Boeing President and CEO Dave Calhoun said: 鈥淲hatever final conclusions are reached, Boeing is accountable for what happened. An event like this must not happen on an airplane that leaves our factory. We simply must do better for our customers and their passengers.鈥 

He added: 鈥淲e are implementing a comprehensive plan to strengthen quality and the confidence of our stakeholders. It will take significant, demonstrated action and transparency at every turn 鈥 and that is where we are squarely focused.鈥 

Boeing announced it had implemented a control plan to ensure all 737-9 mid-exit door plugs are installed according to specifications. 

This included new inspections of the door plug assembly and similar structures at Boeing精东影业 supplier精东影业 factory and on its own production line. 

The company said it had also added 鈥渟ignage and protocol to fully document when the door plug is opened or removed in our factory, ensuring it is reinstalled and inspected prior to delivery鈥.
