An airport ground engineer has tragically died after being sucked inside the engine of a Varesh Airlines B737-500 aircraft.

The incident occurred on July 3, 2024 at Chabahar Konarak Airport (ZBR) in Iran.

According to aviation safety information site JACDEC, maintenance was being carried out on the aircraft精东影业 engine #2 (CFM56). The aircraft, with registration EP-VAF, had just operated as flight VRH-5858 from Mehrabad International Airport (THR).

According to reports, the engine was running with the engine cowlings open when a maintenance engineer entered the safety zone to retrieve tools. In doing so, he was tragically sucked into the engine and was killed instantly.

鈥淚n the morning of July 3rd, one of the specialists of Varesh Airlines, during the pilot精东影业 power-up for reasons requiring investigation and analysis by air accident specialists at the accident site, suddenly found himself in the air suction path of the engine intake and was pulled into the CFM56-3 turboprop engine installed on a Boeing 737, dying instantly,鈥 according to a 鈥榣abor source, who was quoted by aviation media outlet Aviation.be.

As of July 8, 2024, the aircraft remains at ZBR and is being investigated.

The Air Accident Investigation Branch (AAIB) has confirmed in its 鈥楢nnual Safety Review鈥 that 11 people were killed in 10 fatal air accidents in the United Kingdom (UK) during 2023.

The AAIB also announced it received 790 occurrence notifications, compared to 778 in 2022, and opened 25 field investigations – a further 80 investigations were opened through correspondence.

The AAIB said that while the number of deaths was not unusual, they all occurred in the summer months.

None of the fatal accidents in the UK involved large-scale passenger or freight operations and were all centered around 鈥楪eneral Aviation鈥.聽聽

The 10 fatal accidents included seven light aircraft, two gliders and one hot air balloon.

The predominant factor in these fatal accidents was loss of control in flight which 鈥渦sually resulted from low speed near to the ground and the aircraft stalling leading to an incipient or fully developed spin鈥.

鈥淚n 2023 there were 10 fatal air accidents in the UK resulting in 11 deaths. All involved General Aviation. Whilst this number of fatal accidents was not unusual, they all occurred in the summer months and the fatal glider mid-air collision was the first for nine years,鈥 said Crispin Orr, Chief Inspector of Air Accidents.

Orr added: 鈥淟oss of control in flight continues to be the prevalent cause of fatal accidents. The key safety messages to avoid loss of control have been reinforced in revised promotional materials published by the Civil Aviation Authority (CAA).鈥澛

Commercial Air Transport incidents

According to the AAIB精东影业 report the most common accidents involving Commercial Air Transport (CAT) in the UK (and globally) during 2023 were non-fatal runway excursions, ground collisions and tail-strikes.

The AAIB said that 鈥渟erious incidents rarely attract much media attention but are a valuable opportunity to identify safety issues before they become manifest in an accident鈥.

鈥淥ver the last 20 years, two-thirds of the CAT field investigations conducted by the AAIB were into Incidents and Serious Incidents rather than Accidents, with a high proportion yielding Safety Recommendations that have proved to be highly significant in further improving air transport safety,鈥 Orr concluded.

The AAIB also provided support to 48 new overseas investigations where there was a UK interest.聽

The former president of Chile, Sebasti谩n Pi帽era, has died in a helicopter crash in Chile. The accident happened on the afternoon of February 6, 2024, and reportedly occurred in poor weather conditions. The accident has led to an outpouring of grief across Chile and condolences being sent from leaders across Latin America and worldwide.

The helicopter, reported to be a Robinson R44, carrying Pi帽era, 74, and three others plunged into a lake in southern Chile. The former president was pronounced dead shortly after rescue personnel arrived at the scene while the other three passengers onboard the aircraft survived. The Chilean Interior Minister Carolina Toha said the ex-president’s body had been recovered from the lake, near the town of Lago Ranco.

While sources told local media that Pi帽era himself was flying the helicopter, this has not been confirmed by Chilean officials, nor have any details regarding the helicopter’s intended route or destination. Pi帽era was well known to often spend the summers in the Southern Hemisphere in the picturesque lakes region of southern Chile. He was also well-known for frequently piloting his own helicopter.

Current Chilean President Gabriel Boric has declared three days of national mourning, while preparations have begun for a state funeral on February 9, 2024, for the former leader, who served two non-consecutive terms between 2010 and 2022.聽

Pinera was perhaps best known worldwide for taking charge of the dramatic rescue of 33 miners who were trapped underneath the Atacama Desert in 2010. The event became a global media sensation and became the subject of a 2014 movie, 鈥楾he 33鈥.

In Chile, Pi帽era was known as a very successful businessman, and references to him were often paired with the term 鈥榖illionaire鈥. The son of a prominent centrist politician, Pi帽era was a Harvard-trained economist who made his fortune introducing credit cards to Chile in the 1980s. Pi帽era was also a major shareholder in the flagship Chilean airline formerly known as LAN, a Chilean soccer team, and a television station. As of 2024, he was ranked 1,176 on Forbes’ global rich list, with a net worth of $2.7 billion.

In 2010, Pi帽era was involved in another incident involving his R44 helicopter when running low on fuel, he landed it on a rural road in Quilicura Bajo, in the vicinity of Curanipe, about 188 miles (300 km) south de Santiago. Chile. After making a telephone call to ascertain his location and to get fuel, he took off once again.

Pi帽era is survived by his wife Cecilia Morel and his four children.聽聽

The post 鈥楽ociety of the snow鈥: what caused Flight 571 to crash in the Andes? appeared first on 精东影业.]]>
The tragic story of the 1972 Uruguayan Air Force chartered flight that crash-landed in the Andes is one of the most widely-known aviation incidents in modern history.

Through the years, the incident has become known as 鈥淭he Andes flight disaster鈥 or 鈥淭he miracle of the Andes鈥 due to the remarkable story of human survival.

In 1993, Paramount Pictures released a drama film directed by Frank Marshall based on the tragedy called 鈥楢live鈥. The movie starred Ethan Hawke and was narrated by John Malkovich.

The most recent portrayal of the tragedy made its way to Netflix in 2023, 30 years after 鈥楢live鈥 hit the big screen.

The latest adaptation has garnered much praise, with many reviews claiming that it depicted the incident more accurately, given that photography and filming was shot in Sierra Nevada, Spain; Montevideo, Uruguay; Chile and Argentina, and included the actual crash site in the Andes.

The cast was also composed of Spanish-speaking actors, giving the film a more authentic feel than its Hollywood counterpart.

Since 鈥楽ociety of the Snow鈥 was released on Netflix on January 4, 2024, the internet has been rife with articles revisiting the incident, in particular how the survivors managed to stay alive for more than two months. Their story of survival is harrowing and remarkable to say the least.

But as aviation enthusiasts, we have a number of questions about the incident. What really caused the plane to crash? Was it due to bad weather or pilot error? And why did the rescue take so long?

精东影业 takes a closer look at the incident and provides those answers and more.

What aircraft was used?

The aircraft was a four-year old Uruguayan Air Force Fairchild FH-227D (a version of the Fokker F27 Friendship).

It featured a 1.83 m (six ft) stretch over standard-length F27/F-27s, taking the standard 32 seating to 56, with a larger cargo area between the cockpit and the passenger cabin.

The FH-227 was powered by 1678kW Rolls-Royce Dart RDa.7 Mk 532-7 turboprop engines.

A total of 78 FH-227 aircraft were built, 23 of which ended up involved in a crash The Andes flight disaster was the 10th FH-227 crash in 42 years.

The last incident took place in January 2002 when a FH-227E belonging to the aviation unit of Petroproducci贸n, a subsidiary of the Ecuadorian state-owned Petroecuador oil company flew off-course and into the 4,500 meter-high Cerro El Tigre mountain, killing all 21 passengers and five crew members.

And while the Andes disaster can be considered the most high-profile among the crashes, it was not the worst.

Image: Smithsonianmag.com

On June 12, 1982, a twin-engined Fairchild FH-227B (registered in Brazil as PT-LBV) operated by the now-defunct airline TABA (Transportes A茅reos Regionais da Bacia Amaz么nica), crashed into a car park, killing everyone on board.

The plane was conducting a flight between Eirunep茅 Airport (ERN) and Tabatinga International Airport (TBT) when it struck a lighting tower on approach to Tabatinga. The plane crashed into the car park and exploded, killing all 44 on board. The incident was not only the worst aviation disaster in TABA精东影业 history, but the most fatal to involve a Fairchild FH-227.

Timeline and path of flight 571

Flight 571 was a chartered flight carrying members of the Old Christians Club rugby team, along with some of their friends and family, from Montevideo Airport (MVD) to Santiago Airport (SCL) in Chile. In total, 45 people were on board the plane.

The aircraft departed Montevideo Airport on October 12, 1972. However, a storm front over the Andes forced them to stop overnight in Mendoza, Argentina.

The FH-227 was not built to fly higher than approximately 22,500 feet (6,900 meters), so the pilots plotted a course south to the Pass of Planch贸n, where the aircraft could safely clear the Andes.

Last image of the flight before the crash. Image: Reddit

The following day, the flight took off from Mendoza Airport (MDZ) at 14:18 local time.

The plane flew south from Mendoza towards Malarg眉e radio beacon at FL180. The flight radioed Malarg眉e Airport (LGS) with their position and told the airport that they would reach the Planch贸n Pass at 15:21 local time.

The intention was to fly a U-shaped path, first across the pass to the Curic贸 radio beacon and to head north, and then descend towards Santiago.

However, due to a navigational error, the flight crew mistakenly believed that they had already reached Curic贸 and began to descend. The flight精东影业 actual location was still about 80 kilometers east of Curico, in the mountainous Planchon Pass.

This was the fatal error that ultimately led to the FH-227 plummeting into the Andes.

Why did flight 571 crash?

One of the most prominent questions, and one all aviation enthusiasts are likely to want to know the answer to, is this: could the crash have been prevented?

Unfortunately, the answer is yes.

While bad weather was a contributing factor, investigations concluded that pilot error led to the crash.

41-year old Dante H茅ctor Lagurara was a lieutenant colonel in the Uruguayan Air Force, and was co-pilot of the Fairchild 571 aircraft. He was in control of the aircraft at the time of the accident.

Image: sociedaddelanieve.com/

Lagurara failed to see that the instrument readings showed the aircraft was still about 70 kilometers east of Curico. Thinking they had already reached Curico, Lagurara radioed air traffic control, which gave him permission to descend from 18,000 to 11,500 ft.

During the descent, Lagurara regained visual flight conditions and caught sight of the aircraft精东影业 close proximity to the mountains. Despite frantic attempts to gain altitude, it was too late and the FH-227 struck a mountain ridge, shearing off both wings and the tail cone. What remained of the fuselage slid down a glacier at an estimated 350 km/h (220 mph), descending 725 meters (2,379 ft) before ramming into an ice and snow on a glacier.

According to Sociedad dela Nieve, a website dedicated to the plight of all onboard flight 571, Lagurara did not initially die on impact. He was badly injured and trapped in the wreck of the cockpit. The pilot, Colonel Julio C茅sar Ferradas, died upon impact.聽

Lagurara told the survivors, who had been trying to get him out of the wreckage, to find his pistol and shoot him. But the request was denied.聽

Before he died from his injuries on October 14, 1972, Lagurara told the survivors that they had already passed Curico, information that we now know is incorrect. The information caused confusion among the survivors when they were planning which way to trek for help.

Why did the rescue of flight 571 take so long?

The 16 survivors were rescued on December 22, 1972. They managed to stay alive in harsh conditions for 72 days. While that was indeed remarkable, it does beg the question of why it took so long for them to be rescued.

Extreme weather conditions and the erroneous report of its location made it extremely difficult to locate the aircraft. That year, the Andes had a record for snowfall, the most it had in decades.

After losing contact with the FH-227 on October 13, 1972, Pudahuel airport immediately notified the Air Rescue Service stationed at the other airport in Santiago: Los Cerrillos (ULC).

The Chilean Air Rescue Service began its search from the last reported position given by the flight精东影业 co-pilot: the Curic贸 corridor to Angostura and Santiago. The team initially searched in sparse and less populated regions, thinking that if the plane was somewhere in a town or city, news of its crash would have broken out.

After a thorough analysis of all available information, the search team concluded that the plane was not in the reported position when the flight team last contacted the control tower.

Aware of the co-pilot精东影业 miscalculation, the search team collated a list of all the possible areas where the plane could be.

Another challenge that hindered the rescue was that the aircraft精东影业 roof was painted white, making it almost impossible to locate from a distance.

According to Sociedad de la Nieve, the international rescue standard during the time was to search for a plane crash for 10 days. There were a couple of days (October 17 and 18) where the search had to be suspended due to extreme weather conditions which made it impossible to fly over the mountain range.

As days went by without any leads or signs of the missing aircraft, hope of finding the survivors gradually diminished. The search team concluded that if there were survivors from the crash, they would likely perish due to the harsh weather conditions and environment.

Image: sierrahotel.net

On October 21, 1972, the search team announced that it was officially ending its initial search and rescue efforts to locate flight 571 due to the complexity of the operation and extreme weather. The plan was to resume the search in January 1973, this time to search for remains of the passengers.聽

According to the , the survivors were aware of this. The group found a transistor radio that fortunately worked. They listened intently as Chilean news announced that the search had officially ended.

The History Channel reported that the following conversation took place among the survivors:

鈥淗ey boys!鈥 one of them shouted to the rest of the survivors. 鈥淭here精东影业 good news. They鈥檝e called off the search.鈥

鈥淲hy the hell is that good news?鈥 yelled one in response.

鈥淏ecause it means we鈥檙e going to get out of here on our own.鈥

Realizing that survival rested in their own hands, the survivors set off on arduous treks to seek help. It was on the third expedition that they found success.

A total of 16 people survived out of the 45 that were onboard the flight.

A museum dedicated to the crash, Museo Andes 1972, is located in The Old City in Montevideo, Uruguay.

Relics and pieces from the wreckage such as letters, pieces of the fuselage, photographs and memorabilia can be found in the museum.

When Japan Airlines flight JL 516 collided with a Coast Guard aircraft at Haneda Airport精东影业 (HND) runway while landing on January 2, 2023,  the fiery explosion on impact appeared catastrophic and most likely fatal.

The collision claimed five lives from the Japanese Coast Guard精东影业 DHC-8-315Q, but all 367 passengers and 12 crew members of Japan Airlines鈥 Airbus A350 managed to evacuate safely.

The miraculous evacuation from the burning A350 is being credited to the efficient crew, who, despite the chaotic and terrifying conditions, managed to evacuate all 379 people, including eight children under the age of two.

In a report by The Guardian, Japan Airlines claims that all 367 passengers and 12 crew disembarked in under 20 minutes.

Here are factors that could have contributed to a safe and successful evacuation.

The 90-second rule of evacuation

The International Civil Aviation Organization (ICAO) and the require all aircraft manufacturers to demonstrate that all planes can be fully evacuated in 90 seconds.

According to aviation safety site , 90 seconds has been established as the maximum evacuation time because tests have shown that, in a post crash fire, conditions conducive to flashover are unlikely to occur within that time span.

A flashover is the near-simultaneous ignition of most of the directly exposed combustible material in an enclosed area. It精东影业 the instance of a fire spreading very rapidly across a gap due to intense heat.

SKYbrary also explained that the experience of actual evacuations, especially unexpected ones from full aircraft where an abnormal situation occurs suddenly at or soon after landing, indicates that evacuation times usually exceed durations demonstrated for certification purposes.

According to a report by , all of the 367 passengers and 12 crew members of JL 516 exited in just 90 seconds.

Working against the elements and quick thinking

The crew deserves huge acclaim for their efficient and timely evacuation, especially as they were faced with two big hurdles to a smooth evacuation.

First, the fire and impact of the accident damaged the aircraft精东影业 PA system, so the crew had to use megaphones and their own voices to instruct and herd passengers to safety.

Second, when the A350 managed to stop at the runway, the aircraft精东影业 right engine still exhibited sparks with fire growing in the right aft fuselage. credits the crew for making the right decision to deploy emergency slides at the front exits and left rear door.

Calm passengers

As much as the crew are being heralded for saving the lives of everyone onboard JL 516, credit also needs to go to the passengers, who remained calm and composed for much of the time.

A Fly Guy精东影业 Cabin Crew Lounge, a Facebook group for airline and aviation professionals with over 1.3 million members, shared a video clip of what transpired inside the cabin just before evacuation, followed by a clip during evacuation.

It精东影业 noteworthy that passengers remained exceptionally calm in the face of danger. Most of the passengers can see that the aircraft is engulfed in flames, yet no one panicked. Most importantly, they listened to the flight attendants’ instructions and left their belongings in the aircraft.

The Facebook post was captioned with the following:

鈥淣otice how calm the passengers are, even with a fire raging outside. They patiently await instructions from the crew, who are clearly alert and ready for action.

For individuals unfamiliar with evacuation procedures, flight attendants initiate an immediate evacuation only in catastrophic situations, such as water landings or when the aircraft is broken into pieces.

In the video, the crew is likely awaiting instructions from the pilots, who are occupied with critical tasks like turning off engines and identifying usable exits. It is crucial to wait for these instructions to avoid potential dangers, such as passengers using exits near engines or into a fire.

While the flight attendants are aware of the fire, they adhere to the protocol, waiting for evacuation commands. This is precisely what we expect passengers to do during such moments 鈥 remain calm, refrain from grabbing bags, and avoid rushing the doors and yelling to open them.

Crew train yearly for this very situation. Trust their training.鈥

An in-flight safety video that works

Another element that played in the efficient, 鈥榯extbook鈥 evacuation that was performed by the crew, and followed by passengers, was Japan Airlines鈥 in-flight safety video.

While most airlines nowadays focus on tourism, entertainment factor, and using the videos as a marketing strategy, Japan Airlines adhered to the traditional format of its in-flight safety video, using graphic animation to demonstrate what would likely happen if passengers brought personal items during an evacuation.

This is likely to have deterred passengers from insisting on getting their belongings and proceeding to evacuate quickly instead.

In an interview with Channel News Asia, Assistant professor of aviation and integrated systems engineering Shawn Pruchnicki from Ohio State University said that the biggest hindrance to the 90-second rule of evacuation are passengers insisting on taking their belongings.

With JL 516 being evacuated in 90 seconds, Pruchnicki said that it is likely that the passengers followed correct procedures.

New York精东影业 Buffalo Niagara International Airport (BUF) was briefly closed to international flights on November 22, 2023 after an explosion rocked the Niagara Falls Rainbow Bridge at the United States-Canada border.

The explosion occurred at around 11:30 local time, the day before Thanksgiving, considered to be one of the busiest travel days of the year in the US.

Train services between New York and Canada were temporarily halted and officers at Buffalo airport and Niagara Falls International Airport (NIA) went into a heightened state of alert screening cars for explosives.

reported that BUF was closed momentarily, although the airport later issued an update via X (formerly Twitter) stating that the airport remained operational but additional security screenings would be in place for arriving vehicles.

There were initial speculations that the explosion was linked to terrorism. However, the Federal Bureau of Investigation (FBI) and New York governor Kathy Hochul confirmed that the incident had no links to terrorism.

Law enforcement officers told CNN that a man was driving a 2022 Bently at high speed with his wife when the vehicle hit a curb, sending it airborne and hurtling over an eight-foot fence. The vehicle was in flames when it landed, and was completely incinerated.

Footage shared on social media and from surveillance cameras show the car at various angles when it was speeding, flying through the air, and bursting into flames.

Both the husband and wife were reported to have died, and a border patrol employee in a booth also suffered minor injuries.

Reports indicate that the couple intended to watch a KISS concert in Canada, but  headed to a casino in the US when the event was canceled.

Further investigations are still underway and it is not yet not known whether the crash was intentional or an accident.聽
